About the role

On offer is a Permanent fulltime role of Care Navigator – Clinical Nurse.

The role will be working in a Multidisciplinary Team, identifying, enrolling, planning clinical care and guiding eligible patients through the program, enabling and coordinating seamless access to the least restrictive care, in the right place at the right time.

This role will include assessing patients upon admission, liaising with patients, carers, and clinicians during their inpatient stay, assisting with discharge planning and supporting patients to access ongoing outpatient/outreach care aligned to their needs.

 

About you

  • Relevant registration with AHPRA as a Registered Nurse
  • Tertiary qualifications in health care
  • A minimum of 3 years relevant clinical experience in an acute mental health setting
  • Proven ability to develop and implement discharge/care plans
  • A strong commitment to customer service and quality outcomes
  • Ability to build rapport and positive relationships with key stakeholders
  • Highly effective communication and interpersonal skills
  • Demonstrated problem solving and conflict resolution skills
  • Proficiency in the use of digital tools
  • Current drivers licence.

 

Desirable

  • Previous experience in a community mental health setting
  • A strong understanding of mental health conditions, clinical pathways and referral processes. 

 

Key Accountabilities & Position Deliverables

  • Identify patient goals, assess patient clinical needs and have regular check-ins during their hospital stay
  • Attend MDTs where patient discharge planning is discussed
  • Liaise with carers and clinicians throughout the inpatient stay
  • Identify and match patients with appropriate outpatient/outreach services and specialists 
  • Assist with final assessment and formally enrol patients in the program prior to discharge
  • Develop personalised discharge care plans for patients
  • Coordinate patient access to appropriate outpatient/outreach services
  • Deliver appropriate outpatient/outreach mental health services to patients
  • Communicate with patients about their discharge and recovery plan
  • Play a pivotal role in shaping and refining the model of care, including contributing to the program evaluation.

Requirements

  • Must provide a National Police check conducted within the previous 12 months
  • According to the role, Ramsay may require a Working-With-Children check and proof of immunisation against infectious diseases.
