About the Role
North West Private Hospital is seeking a passionate and experienced professional to join our team as a Discharge Planner/Social Worker to support our new 20 bed inpatient rehabilitation unit, opening early 2025.
This role will see the successful candidate join our supportive team of professionals who provide a range of services to assist patients and their families with the emotional and practical impacts of illness, injury and hospitalisation.
In the role, you will be responsible for the discharge planning of an allocated caseload of clients in liaison with medical, nursing, allied health and other health care providers. You will also be required to:
- Assess the discharge planning needs of patients across multiple wards in the hospital
- Advocate for and support patients, carers and their families
- Liaising with family and external agencies for future care planning
- Linking patients to local community services and supports
Your Skills and Experience
- A bachelor’s or master’s degree in social work or current registration with AHPRA as a Registered Nurse
- Experience working as a Discharge Planner/Social Worker in a hospital setting, with experience in acute and/or rehab.
- Ability to work autonomously and also as a team player.
- Ability to problem solve and negotiate with patients and carers to achieve optimal outcomes.
- Adequate clinical knowledge to understand complex patient cases.
- Sound knowledge of My Aged Care system and ACAT referral criteria.
Remuneration
A base hourly rate, based on experience level and qualifications, exclusive of penalties or allowances.
Allied Health Professionals: $40.75 - $60.98 + superannuation, (QLD Health Professionals Grade1-2).
Registered Nurses: $41.89 - $ 54.01 + superannuation (QLD Registered Nurse Level 1.2-1.8)
